Online Modification Submission

THE FOLLOWING MUST BE ATTACHED TO THIS ONLINE REQUEST:

1. No work shall begin until a detailed plan, or set of plans, of the proposed alteration is submitted and approved by the Board of Trustees. The site plan shall indicate what type of alteration is to be constructed showing the exact location on the Lot per the attached sample property survey plan.
2. A detailed description of the characteristics of the proposed alteration (i.e., color, type of material, dimensions, etc.)
3. A copy of the contractor's proposal for all work to be done by a commercial builder.
4. For alterations requiring sealed architect and/or engineering plans, attach a set of the sealed plans, specifically when alteration involves a structural change.

FAILURE TO PROVIDE ALL THE LISTED INFORMATION MAY CAUSE A DELAY IN APPROVAL OF YOUR REQUEST BY THE BOARD.

Exceptions to the requirements listed may be made at the sole discretion of the Board of Trustees of the Villages at Baxter Ridge.

REQUESTOR’S CERTIFICATION: (all property owners must sign below)

1. I understand that nothing contained herein shall be construed to represent that the alterations to land or buildings in accordance with the plans, shall not violate any of neither the protective covenants nor any of the provisions of Building Codes of St. Louis County and the City of Chesterfield to which this property is subject.
2. I understand and agree that no work on the project of this request shall commence until written approval has been received by me from the Board of Trustees.
3. I understand that I may be required to obtain a permit from St. Louis County and the City of Chesterfield for this project, and it is my responsibility to obtain such permit.
4. I understand that review and approval of this request may take up to 45 days. See Second Amendment, Article VI, Paragraph 1 for clarification.
